Accident Plan *
This Plan is FOR THE WHOLE FAMILY. It includes an Accidental Death & Dismemberment Benefit: with up to $50,000. Accident Medical / Dental Expense Benefit: Up to $10,000 which pays for medical expenses resulting from a covered accident up to the maximum benefit amount per covered incident. Subject to a $100 deductible.
   
* All members of the USBA Basic Program are enrolled into the Consumer Alliance USA Association to be eligible to receive these benefits. The benefits are underwritten by an A.M. Best rated insurance company and subject to the exclusions, limitations, terms and conditions of coverage as set forth in the insurance certificate provided in your membership materials and the Policy issued to Consumer Alliance USA.
